SGS Athletics - General Information

Requirements to participate in Athletics and Theater: 

  1. Physical needs to be completed/signed by a physician and turned into the office prior to the first day of practice. Physicals are good for one year. Physicals are not necessary for students to participate in Theater.

  2. ImPACT test: Tests are good for two years.  ImPACT tests are mandatory for basketball, football, wrestling and cheerleading. See below for more details.  

  3. Complete the Google sign-up form for the sport/activity your student wants to participate in (links are below). 

  4. Submit the applicable fees through SchoolCash Online, or bring a check or exact cash to the office. New for the 2025-2026 school year, participation fees must be paid in full prior to the first day of practice. Please see below for more details on fees. 

 

Upcoming ImPACT testing dates at SSMS:

  • August 5, 2025 at 5:30 PM
  • August 11, 2025 at 5:30 PM
  • August 18, 2025 at 4:30 PM

Students will need to bring the signed ImPACT Testing Consent Form and their Chromebook, fully charged. Payment for the ImPACT test is $15 Exact Cash, or Check made out to UCHealth SportsMed.

Fees and Sports Forms

For the 2025-2026 school year, the fees for middle school sports will increase from $90 to $100 per sport per athlete. As an example, if your child participates in a winter and spring sport, the total participation cost is $200. 

The athletic fee covers approximately 20% of the total cost of participating in the athletic program, with the District paying for 80% of the total costs, such as coaches, transportation, uniforms, equipment, and supplies. 

All sport fees must be paid in full and all sport forms, including the ImPACT test (for basketball, football, wrestling, cheerleading) and the physical form (signed by a doctor) must be submitted to the school office before student participation can begin.

Sports physicals should be conducted by the student's pediatrician.
