General Sports Information and Required Forms

Participation Fees:  $75.00 per sport    (contact the SGS office if this is a hardship for your family)             


All sport fees must be paid in full and all sport forms, including the sport contract and the physical form (signed by a doctor) must be submitted in the school office before student participation can begin.


Physical Information:

Sports physicals should be conducted by the student's pediatrician.

Impact testing:

Impact testing is mandatory for athletes in the following sports (however it is recommended for all sports):

  • Basketball

  • Football

  • Wrestling

  • Cheerleading

Group ImPACT test dates at the SSMS Room 186 (Mr. Cuevas' room) at 3:45 pm.:
Monday November 28th @ 3:45 pm 
Wednesday November 30th  @ 3:45 pm

Please contact Justine Jeffers at Justine.Jeffers@uchealth.org to make an appointment (REQUIRED). Students must bring their consent form, demographic page and $15. Test cost is $15 and payment can be made by cash (exact) or by check (payable to UCHealth SportsMed). 

If your child has missed the scheduled group testing sessions at SSMS, Stephanie Stefanelli will be offering one make-up session a month at SSHS (starting in September). This is by appointment only and will be during the first week of each month. You can sign up by emailing stefanelli@ssk12.org



You can also call and schedule an ImPact test with one the below clinics if you cannot make the group testing:

Testing cost is $15 and payment can be made by cash (exact amount) or check.

No gate fees at home events.  Expect about $5 per person for away events.
